• Required Forms--You MUST have the first six (6) forms in this section filled out prior to the start of your season.  They are availoable in Infinte Campus and should be filled out electronically and submitted.
  • OHSAA Physical Form with Concussion form
    All students are required to complete this form before being able to participate in any sport.
  • Madison Athletic Participation Form
    This is our code of conduct for student-athletes and also has eligibility requirements for the high school. This must be completed prior to the start of your season.
  • Student Athletics Participation Waiver
    Must be filled out prior to the start of your season.
  • Sudden Cardiac Arrest
    You and your child must watch the video, and sign the form and turn it in with all the other forms
  • Ohio Concussion Form
    Per Ohio law, all athletes and parents must sign this form each year.
  • Permission to return to participation after a Concussion
    This form must be filled out by a medical professional when you complete the return to participation process.
  • College Grade Check
    For our CCP students- this form will need to be completed to verify your academic progress in your college courses.
  • Transportation in a Private Vehicle
    If you need transportation to/from an athletic event provided by private means, this form must be submitted to Mr. Fisher PRIOR to the event.
